Important Notice
The links below will allow you to download the M-CAAP as an interactive Microsoft Word document that allows you to complete your evaluation using open text fields to enter information. Most of the text is locked in the M-CAAP and cannot be altered; entries can only be made in the open text fields.
The M-CAAP includes instructions throughout for completing each section of the report. These instructions are provided as “hidden” text and will remain hidden until you switch on “view hidden text.” The first resource below is a PDF of the steps to make these instructions visible on your computer. The instructions will not print.

Certification for Massachusetts Providers
All Qualified Diagnosticians contracted or interested in contracting with the Massachusetts Behavioral Health Partnership (MBHP) to conduct ASAP assessments must show proof of completing the M-CAAP training. This is available for Massachusetts providers only. To do this, please sign and email the attestation letter to Sabrina.Askari-Dougherty@carelon.com
